Visa applicants must use the electronic form DS-160, which is available at the following link: https://ceac.state.gov/genniv/
The DS-160 application is a free format which can be accessed through the internet and can be filled out directly by the applicant. This electronic format substitutes the printed DS-156, DS-157, and DS-158 applications. All Nonimmigrant Visa applicants, no matter what the visa category, should fill out this form before they come to their appointment at the Consulate.
Applicants should print the confirmation page that appears after completing the electronic form, and bring this confirmation to the scheduled appointment with the Consulate. You must bring the confirmation page. You do not need to bring a printed version of the rest of the application.

Please keep in mind that all the DS-160 form questions must be completed accurately, otherwise your application will be denied and we will ask you to come back for another interview with a corrected answered form.
The DS-160 form is an application that needs to be completed electronically. For reasons out of the control of the Consulate you may experience technical problems. For this reason, we recommend that you complete your form at least 48 hours before your interview to avoid any inconvenience at the time of your visit to the Consulate.
